Our patient-parent feedback card is given to the patient at every visit. It tells the parent what was done at the appointment, how treatment is progressing (which also makes me keep up with this), and how the patient is doing in oral hygiene. It also contains instructions on elastics, headgear, fixed appliances, and hygiene, along with the time and date of the next appointment.
We have been using this card for several years (Fig. A) and have found that it helps keep parents informed and patients on track. Although it is not an original idea, I designed this specific format.
