New Hints on the Use of Elastic Thread

Anterior Space Closure

The closure of anterior spaces in the Begg technique is usually accomplished by the use of latex elastics either from the tail pin of one cuspid to that of the other (Fig. 1) or from the tail pin of the cuspid on each side to the elastic loop of the archwire on the same side (Fig. 2).

These methods have their disadvantages. Both depend on patient cooperation in the use of the elastics and the cuspid-to-cuspid elastic may impinge on the gingiva or ride up on the incisal edges.

A simple and effective alternative uses elastic thread ligature to accomplish the space closure (Fig. 3). Elastic thread is passed through the intermaxillary circle on the archwire (or caught on the boot hook if that is used). The thread is then passed distal to the bracket and lingual to the archwire. The elastic thread may also be engaged on the tail of the pin in the cuspid if desired.

The use of elastic thread for this purpose has several advantages:

  • 1. It does not depend on patient cooperation.
  • 2. There is no impingement on the gingiva.
  • 3. The elastic thread is not changed, but remains in place for 3-5 weeks depending on the size of the spaces to be closed.
  • Rotating Anterior Teeth

The use of elastic thread ligature in rotation of anterior teeth is often complicated by the tendency for the elastic thread to ride up on the tooth toward the incisal edge. A simple method of overcoming this tendency is as follows (Fig. 4):

  • 1. Pass a piece of .010" ligature wire around the neck of the tooth.
  • 2. Twist the ends of the ligature at the mesiolingual or distolingual corner of the tooth depending on in which direction you want to rotate.
  • 3. The twisted end is cut short and the pigtail is bent gingivally to form a hook.
  • 4. Elastic thread ligature is engaged on the lingual button or eyelet. Both strands of the elastic thread are passed gingival to the hook and then tied to the archwire.
