The best way to learn orthodontics outside of clinical experience is to critically review case reports. Case-based review is the foundation of orthodontic residency education, and it is precisely the method used in this book by Drs. Thomas Southard, Steven Marshall, and Laura Bonner. In an interesting twist, these renowned clinicians and educators have written their text entirely in a question-and-answer format. The reader immediately becomes part of the academic discussion as the authors ask question after question aimed at providing resolution of the presented cases.
The book covers the entire spectrum of problems in the vertical dimension. An opening chapter provides an overview of the fundamental principles of diagnosis and treatment planning, which are applied in the subsequent chapters on dental deep bite, skeletal deep bite, dental anterior open bite, skeletal anterior open bite, and posterior open bite. Each case is beautifully documented with ample records, but there is no mention of bracket types or specific orthodontic manufacturers; the emphasis is all on critical thinking. Some of my favorite clinical pearls include:
• Bonding the lower second molars and, if necessary, placing the archwire under the premolar brackets to promote extrusion of the lower premolars and consequent bite opening in dental deep-bite patients.
• Anticipating a 10-20% increase in overbite during retention of dental deep-bite cases, over-correcting to a shallow overbite and retaining with a maxillary Hawley bite plate and a mandibular Hawley retainer with high acrylic on the lingual of the anterior teeth.
• Treating adult skeletal deep-bite patients who are also mandibular- deficient with tripod mandibular advancement, and leveling the mandibular curve of Spee after surgery.
