Dear Dr. Sheridan,
The section of your article, November 2006, about treating Class II, division 2 cases is what I want to comment about. I find that unlocking the deep overbite often allows the mandible to reposture forward into a better condyle/fossa relationship, thus removing the need for extractions. There are some cases that are crowded enough on the upper arch that extractions are needed, but I would take out second bicuspids in almost all of these cases.
Second bicuspids moderate the crowding but don't move the incisors back as far as removing first bicuspids, which is where you get that undesirable tipped back look of the upper incisors.
Class II, division 2 malocclusion cases have the fewest treatment options and taking out second bicuspids in very selected cases works well for me.
